Revised Action Research Template

I apologize for the image not uploading large. I realize the font is very small. I left the evaluation portion vague because my Site Supervisor and I will potentially change it in the future. We are going to try out different methods while we're evaluating and determine what works best for us. I appreciate all comments and questions.
